All the dwellers from the same rooms - bar and gym, were there but the two mentioned disappeared. One was in a Bar drinking while the second one was in a gym. I don't believe you are losing dwellers, simply you are inable to keep an acurate count of current dwellers and their activities. Open the roster and sort by health, dead one will be on the very bottom and you will have the chance to revive. Do they die, yeah, if an incicent is occurring when you exit or one happens during away time then you can have dead dwellers upon return. You can't train as fast as you can acquire, do they vanish, nope mine never have. I have to turn my radio room to vault only because I have 14 loaded 3 tile level 3 training rooms(at least 2 of all SPECIAL). This happened a second time today but now 3 dwellers have gone missing all at once, I don't know who, but the population again said I have 103 dwellers when I was supposed to have 106 (there was enough capacity for only 106). I even checked every room in my vault wondering if some incident killed someone and I didn't notice, but nothing. As far as I know dwellers don't age and die and nobody died in the wasteland or on any quests. I just logged on one day and saw there was 1 less dweller than there usually was. The first time was early on in my vault and only one person vanished (I knew because of the population counter). I've noticed this twice now that sometimes vault dwellers seem to just vanish.
